Most discharge planners have one concern with placement referrals: will someone actually follow up, or will the family fall through the cracks? Here's exactly what happens after you refer.
Email, phone call, or secure portal — your preference. Give us the family contact, care level needed, and any relevant timeline constraints (discharge date, insurance type, cognitive status). Two sentences is enough. We do the intake from there.
A local CareBridge advisor — not a call center — calls the family directly. We assess care needs, budget, and location preference, then build a shortlist of 2–4 communities that fit. We coordinate tours and stay with the family through move-in. You get a confirmation that the handoff was received.
At 30 days post-referral, we send you a brief family status update: placed, still searching, or other outcome. Optional case-closeout notes available on request for your documentation. Your case stays visible — you're not referring into a black hole.
Why we're safe to refer
We disclose our referral relationship and compensation structure to every family before any community introduction. Families know exactly how we're compensated before we ever suggest a community. That's required under Texas law and we do it by default.
CareBridge is paid by the receiving senior living community upon successful placement. No fee is charged to the discharging hospital, physician practice, social work department, or patient family — at any point in the process.
Families talk to one local advisor, not a rotating queue of commissioned sales reps. We are not A Place for Mom. We do not push volume — we build shortlists based on actual fit: care level, geography, budget, timeline. A family referred to the wrong community is a failure for everyone.
We know which Houston-area communities have strong memory care vs. strong AL programs, which RCHs are appropriate for higher-acuity patients, and which facilities have current availability. We don't work from a national database — we work the Houston market.
Best-fit referrals
Not every case is a fit — we're honest about that. These are the situations where CareBridge adds the most value for your patients and families.
Families facing a sudden cognitive or functional decline need someone who understands the difference between AL, memory care, and residential care homes (RCHs) — and can match accordingly. We do this assessment on every referral, not just the obvious ones.
Discharge pressure is real. When families need to move in days, not weeks, we prioritize communities with open units in the right care tier and coordinate tours fast. We're built for urgency — not just families with 6 months to decide.
Families on the STAR+PLUS Medicaid waiver or VA Aid & Attendance benefit have a narrower set of options. We know which Houston-area ALFs accept STAR+PLUS, which communities accommodate VA benefits, and how to navigate both. We don't send Medicaid families to communities that won't take them.
Some patients need more hands-on care than a standard AL can provide but don't require SNF-level services. Residential Care Homes — licensed for up to 6 residents with 24/7 care — are often the right answer. We know the RCH landscape across the Houston metro and refer accordingly.
