Facility Partners · Houston Metro
Local market expertise, pre-qualified referrals, and families who arrive already understanding your community type, budget range, and timeline.
Submit a Partnership InquiryThe referral process
Every family fills out a structured care assessment covering care type, budget range, timeline, mobility needs, and preferred location. No vague "call us" leads — we capture what matters before we talk to them.
A local CareBridge advisor calls the family, confirms their situation, and builds a shortlist of 2–4 communities. Matching criteria: care type fit, geographic location, budget alignment, availability, and licensing type (AL/MC/RCH).
We introduce the family directly to shortlisted communities. They tour. They choose. Referral fee invoiced only after move-in — not for tours.
What makes our leads different
Every family has completed a structured care needs assessment before we introduce them. No vague inquiries. No cold tire-kickers. They know what they need.
We only introduce families to communities in their preferred area. If a family wants Katy, they don't get options in Humble. Your community receives introductions from families who actually want to be in your market.
We ask about budget before we make introductions. If a family can't sustain your price point, we don't send them to you. Your team doesn't waste tours on families who can't afford your community.
We know whether the family is moving in 30 days or 6 months. We also disclose our referral relationship to every family before introduction, as required by Texas SB 1383 — so everyone's in agreement about how this works.
Fee structure
No surprises. This is how senior placement referral fees work in the industry — and how we apply them.
We only earn when the placement works. That keeps our incentive and yours aligned.
Partnership standards
We only refer families to communities we'd recommend to our own. These aren't unreasonable asks — they're how good communities already operate.
Coverage
We actively serve families and know the senior living market in all 10 of these Houston metro cities. If your community is here, we're already talking to families looking in your area.
Why CareBridge
National placement services send volume. CareBridge sends fit. Here's the difference in practice.
| CareBridge (Local) | National Networks | |
|---|---|---|
| Market knowledge | Knows Houston submarkets — Katy's RCH landscape differs from The Woodlands AL market. We know which communities have quality care vs. glossy lobbies. | National database. Field reps often haven't visited your community in person. |
| Family preparation | Families arrive educated on care types, costs, and what to ask on tour. Less time re-explaining basics on your tour. | Often just a handoff. Families may not understand what AL vs. memory care vs. RCH means. |
| Operator relationship | You know who's calling. Not a call center. Responsive, consistent point of contact. | High staff turnover. Different rep on every inquiry. |
| Exclusive contracts | None required. Partner with us alongside anyone else you work with. | Some require exclusivity or preferred partner fees. |
| Referral transparency | SB 1383 disclosure to every family before introduction. Everyone knows the model. | Disclosure varies by operator and state requirements. |
